Your First Visit
When you come for your appointment please remember to obtain and bring the following:
- If your appointment relates to a WorkCover claim you will need to bring all the claim details together with a copy of the Form 3A document issued by the insurance company
- Referral letter from GP, family physician or other doctor
- Medicare card, DVA card, pension card
- Have your Private Hospital Insurance information with you
- Reports, X-rays, MRIs, CT scans etc. and any other relevant information
